Augusta, Ga. When Dr. Cheryl Newman meets an HIV-positive patient, one of her first questions is whether he knows anyone else with the infection.
The usual response is about dead relatives or friends. "We talk about what that experience was and how, in all likelihood, his experience will be very different," said the infectious-disease specialist at the Medical College of Georgia at Georgia Health Sciences University. "Yes it's the same virus and could have the same outcome if patients do not take advantage of treatment, but in no way does it need to be the same."
Newman, who finished her training about the same time HIV infection began consuming her specialty, also has too many memories of lives lost to HIV. But two-plus decades later, most patients can live long and well taking a single pill daily.
A $3.4 million, three-year grant from the U.S. Department of Health and Human Services for Georgia Health Sciences University will help maintain the new normal. Funds ensure that uninsured patients in the Augusta area get medication and other needed treatment and that the Ryan White Outreach Team continues to reach into the community with rapid testing, education and treatment referrals. GHSU has been funded since 1995 through the Ryan White Comprehensive AIDS Resources Emergency Act to provide these services in 13 nearby Georgia counties as well as Aiken and Edgefield counties in South Carolina.
"We have gotten considerably better at treating the disease," said Dr. J. Peter Rissing, Chief of the MCG Section of Infectious Diseases and the grant's Principal Investigator. "In better than 60 percent of our patients, their virus is undetectable, their immune system is on the rebound, they don't have opportunistic infections. They come see us every six months and we make sure their virus remains undetectable and that their immune system is continuing to mend."
Still, in what Rissing describes as "a dance between the immune system and the virulence of the organism," the best partners are the Ryan White program that enables access, patients who take the improved medicines as prescribed and persistent, hitting-the-streets efforts to get people screened.
Today's drug therapies keep the virus at bay by attacking it from different directions to prevent it from multiplying, Rissing said. Most otherwise healthy patients require only a single pill that combines two or three of these drugs; more rigorous regimens are needed if symptoms such as severe diarrhea and weight loss suggest the virus has damaged the immune system, Rissing said. The virus can become similarly active if medicine isn't taken as prescribed, he noted.
Newman is currently studying a once-a-day pill that prevents the virus from inserting itself into the DNA of cells in the body where it can live and eventually multiply. "We say it takes three active drugs to knock out the virus," she said, optimistic that the drug she is studying will one day enhance the efficacy of one of the two-drug therapies on the market. MCG has participated in the clinical trials of most HIV drugs already in use.
Georgia Health Sciences Health System treats nearly 1,300 HIV-positive patients, most of whom are males age 45-64. The biggest patient increases in the past decade are at both ends of the spectrum. People younger than 23 are among the largest number of newly infected and the older population is increasing because patients are living much longer, said Larry Howell, Grant Manager.
Dorothy Stokes, a patient advocate, works with drug manufacturers and the state to ensure proper medication for uninsured or low-income patients. The state-run AIDS Drug Assistance Program is extremely helpful although new patients face a waiting list. Drug manufacturers are helpful as well, Stokes said.
The Ryan White Outreach Team frequents popular gathering places, including ice cream parlors, nightclubs and, starting within the last six months, various area Walgreens.
The pharmacy chain is proving to be another good partner in outreach, which remains vital since an estimated 25 percent of infected individuals are unaware of their status, Howell said. But experience has shown that when status is known and virus levels are kept low, so are transmission risks, Rissing said.
The 17-year continuum of testing and care has yielded additional benefits, including a cohort of patients large enough to enable the clinical trials that make the latest therapies available to area patients. The GHSU/Charlie Norwood Veterans Affairs Medical Center Psychology Residency Program is one of 23 programs in the U.S. and Canada to offer specialized HIV training as well as one of a handful to offer a yearlong experience that leads to a specialty in medical psychology with an HIV emphasis. Ryan White funds also help support a GHSU College of Dental Medicine residency program that provides dental care to HIV patients.

Xinwei Wang had a hunch that spider webs were worth a much closer look.

Wang, an associate professor of mechanical engineering at Iowa State, studies thermal conductivity, the ability of materials to conduct heat. He's been looking for organic materials that can effectively transfer heat. It's something diamonds, copper and aluminum are very good at; most materials from living things aren't very good at all.
But spider silk has some interesting properties: it's very strong, very stretchy, only 4 microns thick (human hair is about 60 microns) and, according to some speculation, could be a good conductor of heat. But nobody had actually tested spider silk for its thermal conductivity.
And so Wang, with partial support from the Army Research Office and the National Science Foundation, decided to try some lab experiments. Xiaopeng Huang, a post-doctoral research associate in mechanical engineering; and Guoqing Liu, a doctoral student in mechanical engineering, helped with the project.
"I think we tried the right material," Wang said of the results.
What Wang and his research team found was that spider silks ? particularly the draglines that anchor webs in place ? conduct heat better than most materials, including very good conductors such as silicon, aluminum and pure iron. Spider silk also conducts heat 1,000 times better than woven silkworm silk and 800 times better than other organic tissues.
A paper about the discovery ? "New Secrets of Spider Silk: Exceptionally High Thermal Conductivity and its Abnormal Change under Stretching" ? has just been published online by the journal Advanced Materials.
"Our discoveries will revolutionize the conventional thought on the low thermal conductivity of biological materials," Wang wrote in the paper.
The paper reports that using laboratory techniques developed by Wang ? "this takes time and patience" ? spider silk conducts heat at the rate of 416 watts per meter Kelvin. Copper measures 401. And skin tissues measure .6.
"This is very surprising because spider silk is organic material," Wang said. "For organic material, this is the highest ever. There are only a few materials higher ? silver and diamond."
Even more surprising, he said, is when spider silk is stretched, thermal conductivity also goes up. Wang said stretching spider silk to its 20 percent limit also increases conductivity by 20 percent. Most materials lose thermal conductivity when they're stretched.
That discovery "opens a door for soft materials to be another option for thermal conductivity tuning," Wang wrote in the paper.
And that could lead to spider silk helping to create flexible, heat-dissipating parts for electronics, better clothes for hot weather, bandages that don't trap heat and many other everyday applications.
What is it about spider silk that gives it these unusual heat-carrying properties?
Wang said it's all about the defect-free molecular structure of spider silk, including proteins that contain nanocrystals and the spring-shaped structures connecting the proteins. He said more research needs to be done to fully understand spider silk's heat-conducting abilities.
Wang is also wondering if spider silk can be modified in ways that enhance its thermal conductivity. He said the researchers' preliminary results are very promising.
And then Wang marveled at what he's learning about spider webs, everything from spider care to web unraveling techniques to the different silks within a single web. All that has one colleague calling him Iowa State's Spiderman.
"I've been doing thermal transport for many years," Wang said. "This is the most exciting thing, what I'm doing right now."

New research into Earth's primordial oceans conducted by researchers at the Natural History Museum of Denmark at the University of Copenhagen and Stanford University revisits Earth's historical water budget.
The results have just been published in the Proceedings of the National Academy of Sciences.
Water accounts for about ? of a thousandth of Earth's total mass, despite the fact that roughly 70% of the planet's surface is covered by this substance so vital to survival. Indeed, water is a relatively "rare substance" on our "Blue Planet."
Where does water come from?
"One of the absolutely most intriguing things about Earth is that there are oceans of water and that the presence of liquid water has enabled the existence of life on Earth. The researchers have done this by examining 3.8 billion year old minerals from Greenland which are derived from Earth's primordial oceans.
A "minor" loss
"We have managed to reconstruct the isotopic composition of 3.8 billion-year-old seawater using mineral samples from the Isua-rocks in Greenland. The results demonstrate that the young planet's oceans, in relation to those of today, had proportionately more "normal water" than "heavy water" in them. We can explain this difference by the fact that Earth has lost less than ? of its water budget over the last roughly 4 billion years," says Pope.
It may sound like a lot of liquid, but it's a surprise for researchers that Earth's water budget has been so relatively stable for so many years. The new findings concerning the historical development of oceans also support new theories and suggested solutions to "the faint young Sun paradox." Theories challenging the paradox were propounded by a number of researchers from the Natural History Museum of Denmark and Stanford in 2010.
About the faint early Sun paradox
In 1972, renowned astronomer, the late Carl Sagan, and his colleague, George Mullen, formulated "the faint early Sun paradox." The paradox addressed the relative stability of Earth's climate over the 4.5 billion years of its existence in relation to the fact that during the same period, solar radiation has increased by 25-30 percent.
The paradoxical question that arose from the scientists was why Earth's surface, during the planet's infancy, was not covered by ice, when the sun's rays were much weaker than they are today. One possible solution to the paradox, among others, was proposed by the American atmospheric researcher Jim Kasting in 1993. He performed theoretical calculations which showed that 4 billion-years-ago, 30 percent of the Earths atmosphere was composed of CO2. The theory was that the impact of this large amount of greenhouse gas insulated the planet and prevented the oceans from freezing over.
An abundance of atmospheric CO2 however, was not the ice-limiting factor. Instead, a much thinner layer of cloud played a major role in keeping ice at bay. Furthermore, Earth was covered by ocean. This meant that the Sun's relatively unimpeded rays could warm the massive ocean which in turn could store heat and prevent the freezing of its surface, according to the research group from University of Copenhagen and Stanford. This is their current answer to the long-standing riddle.

He has a Masters Degree in Psychology and a Masters in Liberal Arts, with a concentration in Management and Leadership. He is a certified meditation instructor through the American Institute of Health Care Professionals (AIHC) and a certified life coach with Compass Global Group. He is a member of the International Positive Psychology Association (IPPA), and provides training and coaching to assist people in stress and anxiety reduction, and to help them enhance positive emotional experiences.
